“I’d rather have a root canal” is a punchline in English, which tells you how bad the reputation is. That reputation was earned decades ago. With modern anaesthesia and rotary instruments, the procedure is closer to having a deep filling than to the horror story people expect.
The short answer
The treatment itself is not painful, because it is done under local anaesthesia. What most people remember as “root canal pain” is actually the pain of the infected tooth before treatment — which is exactly what the procedure removes.
What actually happens during the procedure
- Anaesthesia — the tooth and surrounding tissue are numbed completely.
- Isolation — a rubber dam is placed so the tooth stays clean and dry, and nothing goes down your throat.
- Access — a small opening is made through the crown of the tooth to reach the pulp chamber.
- Cleaning and shaping — the infected pulp tissue is removed and the canals are cleaned and shaped with fine rotary files, then disinfected.
- Filling — the canals are sealed with a biocompatible material (gutta-percha) so bacteria cannot re-enter.
- Restoration — the tooth is rebuilt, and in most back teeth a crown is placed to protect it from fracture.
Single-sitting versus multiple visits
Most straightforward root canals are now completed in one appointment of roughly 45 to 60 minutes. A second visit is more likely when:
- There is an active abscess that needs to drain and settle.
- The tooth has complex or unusually curved canals.
- The tooth is a molar with four or more canals.
Single-sitting is more convenient, but it is a clinical decision, not a marketing one. A tooth that needs two visits and gets one has a higher chance of failing later.

Molars cost more than front teeth simply because they have more canals to clean. Remember to ask whether the crown is included in the quoted price — for a back tooth it is not optional, it is what stops the tooth splitting six months later.
What to expect afterwards
Mild tenderness when biting for two to three days is normal and settles on its own. Take pain relief as prescribed and avoid chewing hard food on that side until the permanent restoration is placed.
Contact the clinic if you get: severe pain that is worsening after day three, visible swelling of the face, or a crown or temporary filling that comes loose.
Root canal or extraction?
Extraction is cheaper on the day. But losing a molar starts a chain: the neighbouring teeth drift, the opposing tooth over-erupts, and you eventually pay considerably more for an implant or bridge than the root canal would have cost.
Where the tooth is restorable, saving it is almost always the better long-term decision. Where it is badly fractured or has lost too much structure, extraction is the honest answer — and a good dentist will tell you so.

How long does a root canal treated tooth last?
With a proper crown and good hygiene, a root canal treated tooth can last for decades — often for life. The most common cause of late failure is fracture of an uncrowned back tooth.
